Community Connectors Support Hospital Discharge Over Christmas and Easter

Over the Christmas and Easter periods, the Community Connector Team has been working alongside hospital staff to help patients return home safely and feel supported beyond their time in hospital.

During these busy discharge periods, Community Connectors supported people preparing to leave hospital by offering reassurance, practical advice and information about local community support. This included follow‑up phone calls once patients were back at home, helping them feel more confident, independent and connected in the days after discharge.

Hospital staff spoke very positively about the partnership. One discharge manager said the Connectors had made a real difference for patients and described the collaboration as “beneficial” and seamless, with feedback shared more widely across the hospital.
